Embed the finder

What the widget does

• A visitor picks a plan and a service.
• It shows whether prior authorization is required, required for some, not required, referral only, or unknown, who handles it, and the legal decision clock.
• It links to the full answer, sources, and contacts on practice.hsios.org in a new tab.
• The widget collects nothing about patients and saves no plan or service choices. The site may remember scroll positions for navigation. The hosting service may set short-lived security cookies and count page visits.
• Add ?plan= and ?service= to the link to preselect a plan and a service. It resizes itself when you add the script below.
